Registration for COVID-19 Vaccine Appointments

Any member of the public, regardless of where they live, is now eligible to schedule a COVID-19 vaccine appointment at any of the tribe’s outpatient health centers.

During this time, registration documents do not have to be filled out before your vaccine appointment.

Please call 539-234-4099 to schedule your appointment.

CNHS will provide services to those who are determined to be eligible for services under the current Indian Health Service (IHS) regulations and who have provided appropriate documentation outlined in the edibility policy, found in the registration packet. Packets may be submitted in-person or mailed to any Health Services Registration department. Registration Documents can also be scanned and e-mailed to PatientAccess@cherokee.org. The Consent to Treatment form MUST be notarized before mailing or e-mailing to the registration department.
